Transferring from another Agency


Those individuals interested in transferring from another agency must observe the policies and procedure of their current service provider before formally requesting services from New Horizons Supported Services. Further, the individual must then report their intentions to their Service Coordinator. This notification initiates the formal process of transferring.

Once the individuals name has been submitted to New Horizons Supported Services for potential transfer New Horizons Supported Services begins a series of assessments to determine the appropriateness of New Horizons Supported Services for the individual. Information sharing is a vital technique in this assessment period. It affords New Horizons Supported Services the opportunity to analyze data on the individual to determine strengths and weaknesses. This is vital when trying to determine what if any services New Horizons Supported Services employs to serve the applying individual. An observational assessment may be requested also to try an individual in a particular job to see if a match or potential is present.

If the decision is made to serve the individual the individual is required to submit a letter stating that they would like to choose New Horizons Supported Services as their service provider and therefore move their funding from their current provider to New Horizons Supported Services. This letter must be submitted to the individuals Service Coordinator and New Horizons Supported Services. The letter will then be forwarded to DDA who in turn will process the request and make the change.

Request for transfers are subject to many obstacles and therefore can be lengthy, delayed or denied. New Horizons Supported Services makes every effort to accommodate those individuals who express interest in our Community Employment Services program. An open dialogue is maintained with the individual and their Service Coordinator throughout the process. If at any time a concern arises New Horizons Supported Services staff will relay this information to the individual and their caregiver immediately. Further, if the individual is denied services a thorough explanation is provided to the Service Coordinator and the individual.
